This ensures collaboration on your field performance, feedback, and required documentation. Your consent remains in effect until revoked in writing.

Exit Policy Form

TEA Candidate Transfer Form

About this form

The TEA Candidate Transfer Form is required for candidates transferring to another EPP. This form ensures the receiving program can access the candidate’s prior training and progress. Candidates must request the form through the HCCS-ATCP office, which will be sent directly to the new EPP upon request.

Note

This form is required to transfer the candidate's information from one EPP to HCC's ATCP.

 

 

Program Adjustment Form
